Fanuc A06B-6220-H045 spindle amplifier module. Though labeled as a servo amplifier this particular drive is the SPM-45 of 620x series of 200VAC spindle drives from the ALPHA i series. Largest amp with model number is aiSP 45 or aiSP 45-B with a rated output for driving AC spindle motor of 198 amp.

Manufacturer of the series started in 2011 and have gone through a few iterations with the exact same part number which is a slight confusing. Early versions are non -B later are -B, the early versions are not compatible with later controls and the later drives will not work on early controls.
