Fanuc A06B-6220-H015 spindle amplifier module. Though labeled as a servo amplifier this particular drive is the SPM-22 of 620x series of 200VAC spindle drives and motors from the ALPHA i series. Type number is aiSP 15 or aiSP 15-B with a rated output for driving AC spindle motor of 62 amp.

Manufacturer of this series started in 2011 and have gone through a few iterations with the exact same part number which is a slight confusing. Early versions are non -B later are -B, the early versions are not compatible with later controls and the later drives will not work on early controls.
